Form Instructions

  1. Download the selected form
  2. Open with Adobe Acrobat or your favorite reader
    1. Most browsers do not support completing forms online
  3. Complete form with requested information
  4. Digitally sign
    1. Click Digital Signature field
    2. Follow prompts to create a New Digital ID (or use an already created one)
      1. Save to Windows Certificate Store
        1. This will allow you to use this signature to sign other digital documents
      2. Enter your legal name and email address.
      3. Click Save
    3. Click Continue
    4. Click Sign
      1. Do not select Lock Document unless you have already completed all of the other fields
    5. Select location to save a copy of the signed document (the documents folder is recommended)
  5. Print the form and bring with you to your next therapy session
